Our Services include evaluation, treatment planning, therapeutic intervention, and ongoing assessment of children with developmental, physical, neurological, and communication disorders. Services are provided by licensed therapists at our facility located at 404 N. Albany Ave, Tampa, FL 33606.

3. Eligibility

To receive our Services:

  • You must be the parent or legal guardian of the child receiving Services, with authority to make healthcare decisions.
  • The child must be under 18 years of age or have an appropriate transition plan.
  • You must provide accurate and complete information about the child's medical history and current condition.
  • You must have a valid physician referral or prescription where required by state law or insurance.

4. User Responsibilities

As a parent or guardian receiving our Services, you agree to:

  • Provide accurate medical, developmental, and insurance information.
  • Inform us of any changes in the child's medical condition, medications, or contact information.
  • Attend scheduled appointments or provide timely cancellation notice.
  • Participate in treatment planning and follow through with home programs as recommended.
